Q: The instruction on your site:
2.1. Upload "index_files" folder generated with Formoid app. on your php server.
There is no index_files folder. There is a folder called contactform_files. Is that what you mean? There is not index.php file generated by Formiod. There is only contactform.html and contactform.php.

A: Formoid doesn't generate the files with such file names automatically. You have renamed
them (Save HTML...).
Index.php is generated by default.
Nevertheless it doesn't make any influence on the results. Please, follow the instructions.
