Q: A suggestion: If someone picks the 'File Upload' element then you need to add ' enctype="multipart/form-data" ' to the 'form' element. Also, it would be nice if you had a set of 'standard' lists that could be inserted for a dropdown box; e.g., U.S. Cities, Countries, Years, Months, etc.
